Information input in the Contact Forms is collected as a form of verification that you are seeking information and/ or have been a consumer of the professional practice you are reviewing or that you are an approved representative for a practice when purchasing Constasis products or services. You may be asked to opt-in to receiving emails or member rewards mailed to the mailing address you provide. Professionals are encouraged to encourage their consumers to enter reviews to establish the practice’s trend. Constasis may remove reviews that significantly deviates from the established trend of a practice and/or from member accounts who demonstrate inactivity. Professionals who dispute a review that a patient received services from their practice may contact Constasis at email@example.com. Verification of consumer panels will commence through Constasis’ HIPAA secure portal.

If you leave a comment on our Website, you may choose to opt-in to saving your name, email address and website as cookies. How We Use Your Information
How Long We Retain Data
If you leave a comment or review, the comment or review and its metadata are retained indefinitely. This is so we can recognize and approve any follow-up comments or reviews in a timely fashion instead of holding them in a moderation queue. For users that register on our Website, we also store the personal information they provide in their user profile. All users can see, edit, or delete their personal information at any time (except they cannot change their username). Website administrators can also see and edit that information. Accounts with no activity for 24 months are rendered inactive and deleted.
What Rights You Have Over Your Data
If you have an account on our Website or have left reviews or comments, you can request to receive an exported file of the personal data we have on file concerning your profile, including any data you have provided to us. You can also request that we erase any personal data we have on file provided by you. This does not include any data we are obliged to keep for administrative, legal, or security purposes.

We do not receive or store any data from third parties and are not responsible for the practices of websites, applications, or services linked to or from the Constasis site. This includes any information or content on those websites or applications.
